Getting vaccinated is one of many steps you can take to protect yourself and others from COVID-19. Generations Family Health Center is offering the COVID-19 Vaccine.
WHO IS NOW ELIGIBLE TO RECEIVE THE COVID-19 VACCINE?
Everyone 16+ years old who lives, works, or attends school in Connecticut
Please note the vaccine that Generations offers (Moderna) has only been approved for 18 years and older.
WHO IS GENERATIONS VACCINATING NOW?
Due to vaccine and staffing availability, Generations is ONLY vaccinating our patients ages 18 years and older. Please note the vaccine that Generations offers (Moderna) has only been approved for 18 years and older.
WHERE DO 16 AND 17 YEARS OLD GET VACCINATED?
Individuals who are 16 and 17 years old should only sign up for a clinic that offers the Pfizer vaccine, as it is the only vaccine that is currently approved for this age group by the U.S. Food and Drug Administration.
WHERE ELSE CAN I SCHEDULE AN APPOINTMENT FOR THE VACCINE?
Many other healthcare providers are offering COVID-19 vaccinations. Appointment slots are being added every day in the State of Connecticut.
Visit https://covidvaccinefinder.ct.gov/ to find additional COVID-19 vaccination sites, including those that offer the Pfizer vaccine for those between the ages of 16 and 17.
What If I am unable to schedule online?
Those who are unable to self-schedule an appointment due to lack of internet access or access to technology, a disability or a language barrier can call the COVID Vaccine Appointment Assist Line at
877-918-2224. The Assist Line is open seven days a week, from 8 am to 8 pm.

• If your vaccine requires a second dose, it will be scheduled at your first-dose appointment.
• You must live or work in CT to receive the vaccination at Generations.
