Reconsideration of Library Material Form Union Free Public Library, Union CT
To request that the Union Free Public Library reconsider some item in their collection, a Union
Library cardholder may request a copy of this form from the Library Director.
Complete the form in full, sign, and return to the Library Director.
The Library Director shall review the form and the material in question, make a decision, compose a
reply, and send it to the cardholder who has requested the Reconsideration.
Please allow a timeframe of two weeks for the review and reply to be completed. Both the
Reconsideration Form and the Letter of Reply shall go on file at the Library.
